Year 6 After School Cross Country Club

After reading the letter you have been sent very carefully, please fill out the fields below to give your Year 6 child permission to attend Cross Country Afterschool Club.

IMPORTANT: On this form you must indicate if your child will walk home alone (at the later time) or if you will collect from the school yard after the club finishes.

On days when weather conditions are unsuitable, this club will be cancelled – you will receive a text notification before 1pm Wednesday in the event the club is cancelled.

Please ensure to check your school text messages regularly.
